COVID-19 Vaccine Requirement

All Summer Sessions students, including fully online students, are required to be vaccinated against COVID-19 unless they receive a medical or religious exemption. You must provide proof of vaccination or request an exemption before you can register for classes.

Vaccinated Students 

Vaccinated students must verify their vaccination status through a secure student attestation form (UW NetID required). After answering a few questions about your vaccination, you’ll be asked to upload proof of vaccination, such as a vaccination card or other vaccination record.

Students Requesting an Exemption

In addition to the student attestation form, students who are requesting an exemption for medical reasons or deeply held religious beliefs must complete a COVID-19 Medical or Religious Exemption Request. Philosophical exemptions are no longer allowed by Washington state.

Deadlines

If you're enrolling at UW for the first time in summer quarter, you must provide proof of vaccination or submit a request for an exemption before you can register for classes.
